Candidates: How to Apply - Questionnaire

Important Information

All candidates who wish to be considered for federal judicial appointments must complete this Questionnaire. In addition to the completion of this Questionnaire, all candidates must submit the attachments requested in PART 9, PART 10, and PART 11 if applicable. In order for their Application Package to be considered complete, candidates must submit the following to the Office of the Commissioner for Federal Judicial Affairs (OCFJA):

  • One copy of this Questionnaire duly completed. If submitting electronically, candidates must click on the signature field on the last page to digitally sign the document. Alternatively, candidates may print, sign, and scan the completed Questionnaire before submission;
  • One copy of three decisions, legal documents or publications that you have written (See Part 9 - Skills Assessment);
  • Other relevant documentation if applicable (see PART 10 - Personal Suitability and Integrity, and PART 11 - Health and Wellness);
  • A completed Authorization Form for each bar you have been a member of, and;
  • Both completed Background Check Consent Forms
